Transition Planning Tool (into Primary and Secondary School) – Highland Council Psychological Service

Although the title includes “into Primary and Secondary School,” this tool can be used to support enhanced transitions for children with Additional Support Needs at any stage; it contains a selection of topics for staff and parents to consider, plan for, and review in the months leading up to the expected change, i.e., changing educational setting, moving classes, moving on to secondary education, or transitioning out of school altogether.
